Limited availability for evaluations only-all spots already scheduled until my full retirement in December 2022. Contact my office for possible cancellations that would allow a space. I do not maintain a waitlist but ask prospective clients to call monthly to check on the status of currently scheduled evaluations. If you are requesting records please complete Oregon Authorization form above and mail to the address below or fax to the number below. Be sure to complete all highlighted fields including INITIALS-not check marks-in spaces indicated as well as the name AND fax number OR address where the records are to be sent (if client is a minor parent must initial and sign). Note that I do not communicate via email.


Evaluation clients: Please print out and complete applicable packet above as well as Telemedicine Informed Consent if you want a feedback session following completion of your report. Agreement for Parents should be completed if client is minor and parents are divorced. Note that I do no conduct forensic evaluations nor evaluate minor clients whose parents are divorced and engaged in a custody dispute.
